Atraumatic extraction of fractured roots utilizing piezoelectric technology

The removal of fractured roots is often associated with complications in clinical practice. In some cases atraumatic root extraction aims at protecting the buccal bone plate from fracturing during root elevation. In other cases, presence of extensive decay requires flap reflection in order to perform root extraction. Especially when immediate implant placement in the extraction socket is intended, a skilled root extraction should be performed. In these challenging clinical cases the application of piezoelectric technology presents major advantages. A series of clinical cases will be presented, in which utilization of a piezoelectric device and specially designed surgical tips allows the accurate and rapid removal of fractured teeth and roots in an atraumatic fashion. This is usually accomplished without the need for mucoperiosteal flap elevation and without the use of potentially traumatic rotary surgical instruments.
Piezoelectric technology has been extensively applied in orthopedic surgery and neurosurgery.
Only recently have piezoelectric devices designed for oral and maxillofacial surgery appeared in the market. Piezoelectric devices produce ultrasound vibrations and inflict micrometric cuts only on mineralized tissues, leaving the surrounding soft tissues unaffected. Their cutting action includes bone perforation, micrometric osteotomy, horizontal osteotomy, membrane separation from bony walls. Piezoelectric application in oral and maxillofacial surgery offers surgical precision, intra-operative sensitivity and a blood-free surgical site thus providing maximum intra-operative visibility. Piezoelectric technology is a promising tool for osteotomy and osteoplasty in oral surgery, implantology, periodontology and surgical endodontics; is simple to apply and increases the dental treatment quality provided to our patients.
